Nevada Licensed Care Coordinator

A company is looking for an LPN-Care Coordinator to work remotely, coordinating transitions of care for TRICARE beneficiaries. Key Responsibilities Coordinate transitions of care for TRICARE beneficiaries through inbound and outbound calls Communicate with community providers and facilities to ensure care coordination and authorizations are in place Document coordination activities accurately in the Medical Management System (MMS) and participate in quality improvement projects Required Qualifications Current unrestricted Licensed Practical or Vocational Nurse (LPN/LVN) license in the appropriate state or territory Minimum of 2 years' clinical nursing experience; preferred 3 years' clinical experience and 2+ years' Managed Care experience Proficiency in medical data entry and Microsoft Office Utilization of clinical and analytical skills for care coordination needs Ability to work remotely with regular and reliable attendance
