A company is looking for a Triage Registered Nurse (RN). Key Responsibilities Receives calls related to patient care and instructs patients/caregivers on appropriate interventions Assesses patient needs and determines the necessity for nursing visits based on EMR and telephone interactions Communicates with physicians and interdisciplinary teams to coordinate patient care and maintain continuity Required Qualifications Graduate of an accredited school of nursing One to two years of recent acute care experience in an institutional setting One to two years of recent experience in home care preferred RN license for the states served or a compact license required Bachelor's degree preferred, with hospice care experience also preferred
