A company is looking for a Heart Failure Nurse Specialist, RN, California License (CHF). Key Responsibilities Assist in the setup and maintenance of patient charts related to CHF management Collaborate with multidisciplinary teams for inpatient management and continuity of care Deliver comprehensive patient education on CHF, treatment plans, and self-management strategies Required Qualifications RN compact multi-state license and California license required 5+ years of experience treating patients with chronic conditions, including CHF Experience in a CHF bridge clinic or T2D clinic is preferred Experience with remote patient monitoring technology is preferred Ability to work independently in a less structured environment
