A company is looking for a Care Coach I to empower caregivers through support and education. Key Responsibilities Engage and educate caregivers via phone and digital technology to enhance their wellbeing Support multiple caregiver programs focusing on resource navigation and care coordination Collaborate with the coaching team to address caregiver challenges and document interactions Required Qualifications 2+ years of relevant experience in care/disease management with older adults or individuals with disabilities Comprehensive knowledge of Medicaid, Medicare, and Managed Care Strong working knowledge of case management systems and community service delivery Ability to multitask in a fast-paced environment Proficiency in Microsoft Office suite
