Key Responsibilities Assist registered nurses with scheduling and care coordination for patients in the Transplant Care Navigation program Document all activities and update patient medical records while ensuring effective communication with patients and care partners Confirm patient appointments, reschedule missed appointments, and assist in obtaining test results Required Qualifications and Education Associate's degree required 1 - 2 years of related experience, with at least 1 year in a clinical setting preferred Scheduling training and EPIC proficiency preferred Prior remote employee experience strongly preferred Ability to operate basic office equipment and maintain a HIPAA-compliant workspace
