Georgia Licensed Care Manager RN

A company is looking for a Care Manager Transition of Care RN. Key Responsibilities Evaluate member needs through post-discharge assessments and medication reconciliation Develop care/service plans and collaborate with healthcare teams to support member transitions Facilitate education and resources for members and coordinate services to meet their needs Required Qualifications Master's degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing 2 - 4 years of related experience in care management Licensure as LISW, LCSW, LMSW, LMFT, LMHC, LPC, or RN is required Must reside in the Atlanta, Georgia area RN Compact License or Multi-State License is a plus
