Key Responsibilities Outreach to patients for encouragement, goal progress check-ins, and support Review patient status and escalate urgent matters to care team providers Document patient interactions and coordinate care with the patient care team Required Qualifications Current and valid New York State Certified Peer Specialist Certification (NYCPS) One year of Peer Specialist experience with adults aged 18+ with a mental health diagnosis Current or former recipient of mental health services Knowledge in recovery and overcoming challenges from a mental health diagnosis Ability to work independently in a virtual environment
