Patient Care Navigator

A company is looking for a Patient Care Navigator-Care Management. Key Responsibilities Manage patient caseloads through various communication methods and conduct outreach to encourage program participation Assist patients with self-care needs, including scheduling provider visits and accessing community resources Document activities and collaborate with the multidisciplinary clinical care team to evaluate program processes and recommend improvements Required Qualifications, Training, and Education High school diploma or equivalent required; Bachelor's degree preferred MA, LPN, BSW, or Clinical Dietitian with appropriate accreditation/licensure preferred Experience working with patients who have chronic and behavioral health needs Demonstrated experience as part of a multidisciplinary clinical team
