Key Responsibilities Manage level of care reviews for CCC Plus waiver members and newly identified enrollments Collaborate with care coordinators and managers to process LOCERI submissions and reports Conduct monthly audits and process necessary documentation for nursing facility admissions and discharges Required Qualifications 4-year or higher degree in a health-related field with 2+ years of direct experience, or current unrestricted independent licensure as a Social Worker, or LPN with 5+ years of direct experience 1+ years of experience with MS Office, including Word, Excel, and Outlook
