Arizona Licensed Transitional Care Associate

A company is looking for a Transitional Care Associate. Key Responsibilities Facilitates the timely discharge and transfer of clients from hospital care to post-acute settings Implements care plans in collaboration with the healthcare team and communicates transition arrangements Documents interventions in patient medical records and assists patients and families with post-acute care arrangements Required Qualifications Bachelor's degree in social work or related field, or a Licensed Practical Nurse, or a Licensed Respiratory Therapist Knowledge of government/community agencies and resources, including Medicare and Medicaid Effective communication and customer service skills Ability to work flexible hours and weekends on rotation BLS certification required (not required for Insurance Division employees)
