Key Responsibilities Provide outreach to members via phone to assist with care plan next steps and community resources Serve as front-line support for member and provider inquiries, explaining care plan procedures and protocols Document and maintain non-clinical member records in compliance with state and regulatory requirements Required Qualifications High School diploma or GED 1 - 2 years of related experience Knowledge of health plan activities and community resources Understanding of Social Determinants of Health (SDOH) needs Ability to comply with all policies and standards
