Key Responsibilities: Acts as the clinical liaison for patient care referrals, promoting continuity with referral sources Handles communication regarding clinically complex referrals with a customer-friendly approach Completes and processes referral documentation to facilitate the admissions process and ensures compliance with regulatory requirements Qualifications: Completion of an accredited school of professional nursing is required 1 year of experience in home health or hospice care strongly preferred Registered Nurse (RN) licensure is required Clinical knowledge and critical thinking ability for effective patient care coordination Current licensure in the state of practice or a multistate license from the Nursing Licensure Compact (NLC)
