Washington RN Care Manager
A company is looking for a Care Manager (RN). Key Responsibilities Evaluate member needs and recommend care plans to address barriers to accessing appropriate care Develop ongoing care plans and collaborate with providers to meet members' unmet needs Provide education to members and their families on healthcare options and monitor progress towards care goals Required Qualifications Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing 2 - 4 years of related experience RN - Registered Nurse - State Licensure and/or Compact State Licensure required
