Key Responsibilities Collaborate with providers and community pharmacies to process prescription refills and manage medication therapy Document patient assessments and clinical interventions accurately and completely Provide comprehensive patient education to enhance medication compliance and self-management skills Required Qualifications A degree in Pharmacy from an accredited College of Pharmacy Completion of an accredited PGY1 pharmacy residency program or three years of pharmacy experience Current and unrestricted pharmacy licensure in the applicable state of practice within 90 days of hire date Good organizational and computer skills Experience in ambulatory care setting is preferred
