Key Responsibilities Engage high-risk patients post-discharge and ensure follow-up appointments with Primary Care Providers (PCP) are scheduled Collaborate with care team members and external vendors to facilitate patient care and close care gaps Request medical records from various healthcare systems to ensure continuity of care Required Qualifications, Training, and Education 3+ years of experience in a medical office or healthcare setting 2+ years working as a Licensed Practical Nurse (LPN), Medical Assistant (MA), Pharmacy Tech, or Community Health Worker (CHW) preferred Comfort with technology, including the Microsoft suite of products Prior experience using electronic health records for data capture and reporting
