A company is looking for a Member Health Assessor with a 4-day weekday and 1-day weekend schedule. Key Responsibilities Engage with members to conduct health risk assessments and establish professional relationships Coordinate logistics to support care plan goals and verify member eligibility and health status Assist in the development of care plans and provide health education and wellness materials Required Qualifications High School Diploma or General Education Diploma (GED) is required A minimum of two (2) years of experience working with at-risk populations in community settings is preferred Proficient in Microsoft Office, including Outlook, Word, and Excel Experience working within diverse cultures and with various stakeholders Ability to maintain confidentiality and demonstrate ethical conduct in all interactions
