A company is looking for a Member Health Assessor to engage with members and conduct health risk assessments. Key Responsibilities Conduct health risk assessments by gathering information from members and stakeholders Coordinate logistics to support care plan goals and assist with health education and wellness materials Maintain documentation and contribute to care plan development based on assessment outcomes Required Qualifications, Training, and Education High School Diploma or General Education Diploma (GED) is required Minimum of two (2) years of experience working in community settings with at-risk populations is preferred Proficient in Microsoft Office, including Outlook, Word, and Excel Experience working within diverse cultures and with various stakeholders Ability to maintain confidentiality and demonstrate ethical conduct
