A company is looking for a part-time RN Care Manager to support maternal health patients through care coordination and remote monitoring. Key Responsibilities Monitor patient data trends and identify high-risk cases requiring intervention Perform clinical assessments and provide direct patient education on maternal health concerns Support care coordination by assisting patients with referrals and scheduling follow-ups Required Qualifications Active, unrestricted RN license in the U.S. (multi-state compact license preferred) Minimum of 3 years of clinical experience in maternal health, OB/GYN, or case management Experience in telehealth or digital health platforms is preferred Proficiency with EHRs and care management platforms Bilingual (Spanish preferred)
