A company is looking for an Associate Care Manager. Key Responsibilities Engage with patients through messaging, phone calls, and telehealth to support care plan adherence Monitor patient data and follow up with those who have not submitted readings Educate patients on using digital health tools and measuring vital signs Required Qualifications Experience in care coordination, patient engagement, or remote patient monitoring (RPM) is preferred Background as a Medical Assistant (MA), Certified Nursing Assistant (CNA), or similar healthcare role Proficient with digital health tools, EHR systems, and telehealth platforms Ability to work independently in a remote environment Bilingual (Spanish preferred) is a plus
