Key Responsibilities Develop and operationalize value-based care programs and initiatives Act as a subject matter expert on patient attribution and its implications in value-based care contracts Collaborate with stakeholders to enhance provider-facing tools and manage complex needs across markets Required Qualifications Bachelor's degree in Healthcare Administration, Public Health, Business Administration, or related field (Master's preferred) Minimum of 4 years of experience in healthcare, preferably in value-based care Knowledge of care center workflows and EMR systems (Athena and Epic preferred) Advanced knowledge of value-based care concepts and project management tools Proficiency in Microsoft Excel and presentation development tools
