Telephonic Care Navigator
A company is looking for a Telephonic Care Navigator to provide support and outreach to members remotely. Key Responsibilities Conduct telephonic outreach to members and assist with scheduling appointments Educate members on care gaps and connect them with community resources Maintain confidentiality of health information and ensure adherence to program guidelines Required Qualifications High School Diploma / GED or equivalent work experience At least 1 year of call center or telephonic customer service experience 1+ years of experience in an office setting using phone and computer 1+ years of healthcare, insurance, or community outreach experience Ability to work from home with access to high-speed internet
