A company is looking for an AZ Licensed Clinician: LMSW, LCSW or LMFT in Behavioral Health. Key Responsibilities Provide customized care and treatment plans for patients in assigned Care Pods Coordinate behavioral health care with primary care providers and monitor patient symptoms Document patient progress and treatment recommendations in electronic health records Required Qualifications, Training, and Education Master's Degree in Social Work, Counseling, or a related field Licensed as an associate clinician under supervision or as an independent clinician in their state Ability to complete cross-licensing in Arizona if the company covers the cost Experience with behavioral health screening tools is preferred Fluency in English; Spanish proficiency is highly preferred
