A company is looking for a Care Guide Representative. Key Responsibilities Acts as the central facilitator to patients and supports them through all stages of care Identifies and resolves barriers to patient care and ensures connections to necessary resources Develops patient-driven goals and utilizes behavior change techniques to empower patient participation in their health Required Qualifications Must be 18 years of age with relevant education and/or experience 0 to 2 years of experience in customer service, health care, or community-based role High school diploma or GED preferred Bachelor's degree in health care is a plus
