A company is looking for a Clinical Pharmacist (Remote) to support patients in promoting health and managing chronic diseases. Key Responsibilities Lead a patient-centered disease management program for chronic conditions while promoting medication adherence Collaborate with healthcare providers to ensure optimal patient care and conduct Medication Therapy Management (MTM) Analyze patient medication profiles and provide education on medication use and health management Required Qualifications Doctor of Pharmacy (PharmD) and a current pharmacist license in good standing Clinical experience in Medication Therapy Management and pharmacist-led Clinical Disease Programs Knowledge of healthcare industry practices and HIPAA regulations Ability to prioritize tasks and manage multiple projects independently Proficient with Microsoft Office Suite and capable of learning new computer systems
