Key Responsibilities Provide customized care and treatment plans for patients in assigned Care Pods Coordinate behavioral health care with primary care providers and monitor patient symptoms Document patient progress and treatment recommendations in electronic health records Required Qualifications and Education Master's Degree in Social Work, Counseling, or a related field Licensed to practice as an independent clinician (LCSW/LICSW, LMHC, LMFT, LPC, or equivalent) Ability to complete cross-licensing in Texas and/or Florida if required Fluency in English and Spanish, including medical terminology Proficiency in evidence-based treatment approaches is highly preferred
