A company is looking for a Complex Discharge Planner II (Nurse or Social Work). Key Responsibilities Assess, plan and coordinate safe, effective and clinically appropriate transition plans Collaborate with internal and external teams for clinical case review and planning Establish and maintain collaborative relationships with hospitals and other identified stakeholders Required Qualifications Bachelor's or graduate degree from an accredited school of nursing or social work 4+ years of nursing or social work experience 1+ years of care coordination or discharge/transition planning experience Current state's RN license or LICSW required Knowledge of behavioral health impacts on physical health preferred
