A company is looking for a MLTSS Clinical Care Associate. Key Responsibilities: Assist with scheduling and coordination of services in the member's Plan of Care Provide telephonic assistance and follow-up to ensure timely member engagement Document interactions and coordinate the delivery of care based on members' needs Required Qualifications: High School Diploma/GED required Preferred healthcare industry experience Minimum one (1) year experience in home care, discharge planning, or case management Minimum one (1) year experience in the managed care industry preferred Knowledge of managed care principles and standards of practice for care managers
