A company is looking for a Telephonic RN Nurse Care Manager. Key Responsibilities Manage a caseload of high-risk patients, coordinating their care and addressing barriers Prioritize patient needs and collaborate with a multidisciplinary team to develop individualized care plans Implement and evaluate comprehensive clinical care management plans to reduce unplanned hospitalizations Required Qualifications, Training, and Education Active R.N.
