Ohio Telephonic Care Navigator
A company is looking for a Telephonic Care Navigator to provide support to members through telephonic outreach.
Key Responsibilities
Conduct telephonic outreach to members, educating them on care gaps and scheduling provider appointments
Assist members with social determinants of health and connect them to community resources
Maintain compliance with metrics and ensure confidentiality of health information
Required Qualifications
High School Diploma / GED or equivalent work experience
1+ years of call center and/or telephonic customer service experience
1+ years of professional experience in an office setting using telephone and computer
1+ years of experience in healthcare, insurance, or social work/community outreach
Ability to work from home with access to high-speed internet
