Key Responsibilities Conducts telephonic outreach to DSNP/CSNP members and educates them on care gaps Assists members with social determinants of health and connects them to community resources Maintains compliance with metrics, provides excellent customer service, and ensures confidentiality of health information Required Qualifications High School Diploma / GED or equivalent work experience Must be 18 years of age or older 1+ years of call center and/or telephonic customer service experience 1+ years of professional experience in an office environment using telephone and computer 1+ years of healthcare/insurance or social work/community outreach experience
