Tennessee Bilingual Care Coordinator

A company is looking for a Bilingual Care Coordinator to provide care coordination for Medicaid, Medicare Advantage, and Dual Eligible Special Needs Plan members. Key Responsibilities Conduct outreach to educate members about program benefits and facilitate their engagement Assess member status and develop personalized care plans addressing their goals Manage a caseload, ensuring timely communication and coordination of care with providers Required Qualifications 2+ years of experience as a care coordinator, case manager, community health worker, pharmacy technician, or social worker Fluency in English and Spanish Must be located in Tennessee with a dedicated home office for remote work Experience working with Medicaid and/or Dual Eligible patients preferred LPN or RN preferred, but not required
