A company is looking for a Hospice Afterhours Triage Nurse - PT Weekends. Key Responsibilities Assess physical, environmental, and emotional factors to determine hospice needs Utilize EHR to develop recommendations and provide patient/caregiver education Collaborate with pharmacies and agencies for effective patient management and assign visits to on-call staff Required Qualifications Current, unencumbered RN license in the assigned state One year of experience as a registered nurse Current CPR certification Knowledge of the needs of terminally ill patients and caregivers Comfortable with technology and willing to obtain additional RN licenses in other states
