Key Responsibilities: Completes telephonic outreach to members and educates them on gaps in care Assists members with social determinants of health and links them to community resources Maintains schedule adherence and ensures compliance with health information regulations Required Qualifications: High School Diploma / GED or equivalent work experience 1+ years of call center and/or telephonic customer service experience 1+ years of professional experience in an office setting using telephone and computer 1+ years of Healthcare/insurance or community outreach experience Ability to work from home with high-speed internet access
