Care Management Specialist

A company is looking for a Care & Wellness Education & Coaching Specialist. Key Responsibilities Provide person-centered, strengths-based care management services to long-term care insurance policyholders Conduct ongoing assessments, establish goals, and research local resources for policyholders and their families Educate policyholders and caregivers on medical conditions, daily living aids, and resources to support aging in place Required Qualifications, Training, and Education At least 2 years of supervised care management experience with older adults and their families 5-7 years of professional experience in settings such as home care, hospice, or nursing homes College degree in a relevant field or equivalent work experience Strong knowledge of care, treatment, and community programs for individuals with functional and cognitive impairments Ability to work independently and in a team environment while managing multiple responsibilities
