Key Responsibilities Manage inbound and outbound telephone communication and data processing according to standards Enter and verify member information in the Care Transitions platform and assist in care coordination Complete administrative documentation for member records and attend Care Transitions meetings as needed Required Qualifications High School Diploma / GED or equivalent work experience Must be 18 years of age or older 1+ years of data entry experience 1+ years of experience with computers in an office environment Ability to work any of the 10-hour shift schedules during normal business hours
