Clinical Pharmacist - MN Licensed
A company is looking for a Clinical Pharmacist to provide clinical support for safe and effective medication use and access. Key Responsibilities Review and assess individual member clinical cases for medication-related opportunities and recommendations Provide consultative medication-related advice directly to medical plan members and interdisciplinary care management team members Develop and provide interdisciplinary pharmacy-related education materials and participate in pharmacist engagement program enhancements Required Qualifications Active, unrestricted license to practice as a Pharmacist in the employee's state of residence Able to obtain active, unrestricted licensure to practice as a Pharmacist in Kansas within 6 months of hire 3+ years of experience as a licensed Pharmacist 2+ years of direct patient/member interaction 2+ years of experience in Health Plan (Managed Care) or hospital or long-term care experience
