Key Responsibilities Develops and facilitates complex care management activities and personalized care plans for members Coordinates with providers and community resources to address member needs and barriers to care Monitors member progress and updates care plans as necessary to meet their goals Required Qualifications Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing 2 - 4 years of related experience RN - Registered Nurse - State Licensure and/or Compact State Licensure required
