A company is looking for a Provider Chart Reviewer. Key Responsibilities Review patient medical records to ensure accurate clinical documentation Evaluate issues to signal providers for necessary clarifications and explore records for potential diagnoses Collaborate with a team to maintain standardization and meet compliance and productivity goals Qualifications Actively-licensed Nurse Practitioner (NP) or Physician Assistant (PA) with any state license 3+ years of clinical practice experience, including medical record review/navigation Experience with HCC coding practices or Medicare Risk Adjustment preferred Basic understanding of CMS compliance practices Ability to work 40 hours per week, with a preference for at least 2 years of experience in Primary Care
