Mississippi Licensed Care Manager RN

A company is looking for a Care Manager NICU - RN. Key Responsibilities Engage members and their support systems through assessments and care planning to improve health outcomes Facilitate inter-disciplinary care team meetings and coordinate care to meet member needs Monitor and document care coordination activities, ensuring compliance with standards and regulations Required Qualifications and Education Nursing degree from an accredited program or a Bachelor's degree in a health care field A minimum of three years of experience in nursing, social work, or related healthcare fields Preferred experience in Medicaid and/or Medicare managed care Current unrestricted clinical license as a Registered Nurse, Social Worker, or Professional Clinical Counselor Case Management Certification is highly preferred
