North Carolina Licensed Case Manager
A company is looking for a Case Manager. Key Responsibilities Coordinate care, resources, and services for members to achieve optimal clinical and resource outcomes Develop and monitor member-centric care plans, addressing barriers and utilizing community resources as needed Document all aspects of care management from assessment to evaluation of outcomes Required Qualifications RN, RD, or LCSW with 3 years of clinical and/or case management experience Valid and applicable clinical license (NC or compact multi-state licensure) Additional specialty certification (i.e., CCM, CDCES) may be required for some roles Must obtain relevant certification within 2 years of employment if applicable
