A company is looking for a Remote Case Manager, RN. Key Responsibilities Collaborate with a health care team to monitor utilization patterns and facilitate appropriate health care delivery Conduct comprehensive assessments of members' health status, including medical records and support needs Coordinate treatment plans and discharge plans with primary care physicians and family members Required Qualifications RN or LPN with a North Carolina license required Prior telephonic case management experience is a plus 3+ years of inpatient care experience Critical care experience, including med-surg and ICU Must reside in the EST time zone
