Key Responsibilities Provide fully remote virtual serious illness care management to patients and families Consult with medical staff and perform virtual assessments of patient needs Document interactions in the electronic medical record and support discharge planning Required Qualifications, Training, and Education Minimum of 3 years of recent clinical experience in hospice, palliative care, or acute inpatient care Master's or Doctor's degree in Nursing from an accredited Family or Adult Nurse Practitioner Program National certification, licensure, and prescriptive authority from the appropriate governing body Basic Life Support certification required by the date of hire Advanced Certification in Hospice and Palliative Nursing (ACHPN) must be obtained within 3 years of hire
