Oregon Licensed Behavioral Health Navigator
A company is looking for a Behavioral Health Navigator (QMHA) to coordinate care for behavioral health needs remotely in Oregon.
Key Responsibilities
Coordinate care between members, primary care, emergency departments, and behavioral health providers
Connect members to community resources, including housing and employment services
Monitor and document progress of care coordination plans and assist with care management duties
Required Qualifications
Two years of experience in healthcare administration or case management
Strong knowledge of community resources and behavioral health providers
High School diploma or equivalent
Qualified Mental Health Associate (QMHA) certification or ability to obtain
Bilingual in Spanish preferred
