Peer Navigator

A company is looking for a Peer Navigator to provide support to trauma survivors in their recovery journey. Key Responsibilities Serve as the primary care navigator for assigned patients, guiding them through all phases of care Conduct individual and group peer mentor sessions to empower patients in their recovery Coordinate care between healthcare providers and manage referrals to ensure patient needs are met Required Qualifications Lived experience with accessing mental health treatment Fluency with PTSD or related experiences Training as a peer mentor (training can be provided for preferred candidates) Healthcare coordination experience preferred High School degree or higher; bilingual in Spanish is a plus
