Key Responsibilities Manage data and registries for special populations, facilitating care coordination and quality improvement initiatives Provide direct member care through outreach, assessments, and education on health and recovery options Support pilot projects and administrative tasks as directed by leadership, contributing across medical, mental health, and social care areas Required Qualifications Registered Nurse (RN) or Licensed Practical Nurse (LPN) for Medical Focus; Licensed Clinical Social Worker for Mental Health Focus; or relevant certification for Social Care Focus At least 5 years of experience in direct patient care or supervision in relevant fields Strong understanding of care coordination, case management, and healthcare regulations Proficiency with electronic medical records and documentation tools Ability to work flexibly, including adjusting hours to meet member needs
