Key Responsibilities Assist members by completing health assessments and implementing care plans to improve health and wellness Provide telephonic support and coaching for members on wellness care plans and programs Monitor wellness plans and member outcomes, providing recommendations to reduce health risks Required Qualifications Associate's degree and 2-4 years of related experience Registered Dietitian, Physical Therapist, Respiratory Therapist, RN, or Master's in Behavioral Health required Health Coach Certification is required Strong preference for candidates with Registered Dietitian licensure
