Registered Nurse Care Manager
A company is looking for a Care Manager (RN) to develop and facilitate complex care management activities for members with physical needs. Key Responsibilities Evaluate member needs and recommend care plans to address barriers to accessing appropriate care Coordinate with providers and community resources to ensure comprehensive care for members Monitor member progress and update care plans as necessary to meet individual goals Required Qualifications Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing 2 - 4 years of related experience in care management Registered Nurse (RN) state licensure required Strong background in case management and behavioral health management
