A company is looking for a Director of Care Management. Key Responsibilities Develop and execute the long-term strategy for care management services to optimize patient outcomes Drive transformation in care delivery by integrating clinical, behavioral, and social determinants of health Build and sustain external partnerships to enhance care continuity and address service gaps Required Qualifications RN with specialization/experience in family medicine, internal medicine, and/or psychiatry 5+ years of experience as a Case/Care Manager with appropriate licenses and certifications 5+ years of experience leading a team 3+ years of experience with patients having complex chronic health conditions Experience in inpatient care settings preferred
