Key Responsibilities Manage inbound notifications related to transitional care events from various sources Conduct outreach to facilities for information on admission, discharge dates, and plans Request and obtain medical records, and schedule post-discharge appointments for patients Required Qualifications High school diploma required; college degree preferred Strong computer skills, including knowledge of Microsoft Excel and electronic medical records Experience in a customer service setting, preferably in healthcare Fluency in Spanish, Polish, Russian, or other relevant languages is a plus US work authorization is required
