A company is looking for a Transition Care Coordinator/RN/LCSW/LPC/LPN. Key Responsibilities Facilitate safe and effective transitions in collaboration with care teams and facilities Participate in interdisciplinary care team meetings Coordinate care for individuals, particularly those with Medicaid or CCC Plus populations Required Qualifications, Training, and Education Associate's, Bachelor's, or Master's degree in Health & Human Services or related field Licensed as RN, LPN, LCSW, or LPC in the state of Virginia Minimum of two years of case management or long-term care experience Experience in nursing, social work, or human services field One year of experience working with Medicaid or CCC Plus populations
