Key Responsibilities Provide telephone triage assessment and health advice to Primary Care patients using approved protocols Document calls in a timely manner according to department standards for legal and statistical purposes Ensure performance standards are met while delivering superior customer service to internal and external customers Required Qualifications Active RN license 3+ years of experience in a clinical setting (Med/Surg, critical care, ER, etc.) Experience in disease management, home health, discharge planning, utilization review, and patient education Experience in telephonic nursing Case management experience preferred
